Balloon atrial septostomy procedures for cyanotic congenital heart defect in Sanglah Hospital

Abstract

Balloon atrial septostomy (BAS) is a technique used to enlarge a hole between right and left atrium. Balloon atrial septostomy is an initial life saving treatment in certain cyanotic congenital heart defect especially transposition of great arteries-intact ventricular septum (TGA-IVS) and pulmonary atresia-intact ventricular septum (PA-IVS). This procedure aimed to create an atrial septal defect that will enhance bidirectional mixing of pulmonary and systemic venous blood, hence improving oxygen saturation in this type of heart defect. Complications of BAS consist of arrhythmia, heart block, valve dysfunction, myocardial ischemia, neurodevelopmental abnormalities, stenosis or occlusion of coronary artery. In our centre, during April 2015 - June 2016 there were 4 BAS procedures and 1 re-ballooning. Range of ages was 1-15 days in BAS patient. The NuMedZ-5TM® (NumedInc, New York, U.S.A) balloon catheter number 13.5 was used. Diameter of atrial communication and oxygen saturation was increased significantly after procedure.

 

Balloon atrial septostomy (BAS) adalah tehnik yang digunakan untuk memperbesar defek antara atrium kanan dan kiri. Balloon atrial septostomy adalah terapi awal pada defek jantung bawaan sianosis tertentu terutama pada transposition of great arteries-intact ventricular septum (TGA-IVS) dan pulmonary atresia-intact ventricular septum (PA-IVS). Prosedur ini bertujuan untuk memperbesar defek septum atrium yang akan meningkatkan pertukaran darah vena pulmonal dan sistemik, sehingga meningkatkan saturasi oksigen. Komplikasi BAS terdiri dari aritmia, jantung berhenti berdetak, disfungsi katup, iskemia miokard, kelainan perkembangan saraf, stenosis atau oklusi arteri koroner. Di RSUP Sanglah, selama bulan April 2015 - Juni 2016 ada 4 prosedur BAS dan 1 prosedur dilakukan ballooning ulang karena terjadi stenosis pulmonal. Rentang usia pasien adalah 1-15 hari pada pasien BAS. Balon yang digunakan untuk BAS adalah NuMedZ- 5TM® (NumedInc, New York, U.S.A) nomor 13.5. Diameter atrium dan saturasi oksigen meningkat secara signi kan setelah prosedur.
