Hubungan Kuat Antara Disabilities of Arm, Shoulder and Hand Score dan Modified Mayo Wrist Score Dipengaruhi oleh Radius Union Scoring System dan Kadar C-Reactive Protein pada Pasien Fraktur Distal Radius dengan Tatalaksana Konservatif

Abstract

Background: Distal radius fracture is among the most common fractures in the upper extremity area. Conservative management of these fractures often results in good outcomes, which can be measured using several scoring systems and is often associated with a controlled inflammatory response. This study aims to determine the correlation between the Modified Mayo Wrist Score (MMWS) Radius Union Scoring System (RUSS) and the Disabilities of Arm, Shoulder and Hand (DASH) gold standard scoring system, as well as looking at the correlation of the inflammatory marker C-Reactive Protein (CRP) with the scoring system.

Methods: This observational study includes 35 distal radius fracture patients treated conservatively with a cast maintained for six weeks, followed by an x-ray examination to assess the RUSS score and blood sampling at week 9 to measure CRP levels. At week 12, the DASH and MMWS scores were evaluated. All analyses used SPSS v.26 with a p-value <0.05 considered significant.

Results: This study sample consisted of 17 men and 18 women, with a median age of 36 and 44.5 years, respectively. Median RUSS score, DASH, MMWS, and CRP level were 6 (IQR 1), 2.5 (IQR 7.5), 80 (IQR 25), and 0.75 (IQR 0.55). MMWS and RUSS scores showed a significant inverse correlation with DASH scores (r = -0.919; p<0.05 and r = -0.826; p<0.05). DASH and MMWS scores significantly correlated with CRP levels (r = 0.779; p<0.05 and r = -0.837; p<0.05).

Conclusion: MMWS and RUSS scores significantly correlate with DASH scores, so their application can be applied in clinical practice. DASH and MMWS scores also had a significant correlation with CRP levels.

 

Latar Belakang: Fraktur distal radius merupakan salah satu fraktur yang paling sering terjadi pada daerah ekstremitas atas. Tatalaksana konservatif pada fraktur ini sering menghasilkan luaran yang baik, yang dapat diukur menggunakan beberapa sistem skoring, serta sering dikaitkan dengan respon inflamasi yang terkontrol. Penelitian ini bertujuan untuk menentukan korelasi antara Modified Mayo Wrist Score (MMWS) Radius Union Scoring System (RUSS) terhadap sistem skoring baku emas Disabilities of Arm, Shoulder and Hand (DASH), serta terhadap kadar C-Reactive Protein (CRP).

Metode: Studi observasional ini melibatkan sebanyak 35 fraktur distal radius yang ditatalaksana secara konservatif dengan gips yang dipertahankan selama 6 minggu, diikuti dengan pemeriksaan rontgen untuk menilai skor RUSS, dan pengambilan sampel darah pada minggu ke-9 untuk mengukur kadar CRP. Pada minggu ke-12 dilakukan penilaian skor DASH dan MMWS. Seluruh analisis menggunakan SPSS v.26 dengan nilai p<0,05 dianggap signifikan.

Hasil: Sampel penelitian terdiri dari 17 orang laki-laki dan 18 orang perempuan, dengan median usia masing masing adalah 36 dan 44,5 tahun. Median skor RUSS, DASH, MMWS, dan kadar CRP masing-masing adalah 6 (IQR 1); 2,5 (IQR 7,5); 80 (IQR 25); dan 0,75 (IQR 0,55). Skor MMWS dan RUSS menunjukkan korelasi inversi yang signifikan terhadap skor DASH (r = -0,919; p <0,05 dan r = -0,826; p <0,05). Skor DASH dan MMWS memiliki korelasi yang signifikan terhadap kadar CRP (r = 0,779; p <0,05 dan r = -0,837; p<0,05).

Simpulan: Skor MMWS dan RUSS memiliki korelasi yang signifikan dengan skor DASH, sehingga penggunaannya dapat diterapkan dalam praktek klinis. Skor DASH dan MMWS juga memiliki korelasi yang signifikan dengan kadar CRP.
