Validitas diagnostik Blunt Abdominal Trauma Scoring System (BATSS) pada trauma tumpul abdomen di RSUP Sanglah Denpasar, Bali

Abstract

Pendahuluan: Trauma abdomen merupakan salah satu penyebab kematian ke-3 pada pasien trauma dan dapat ditemukan sekitar 7–10% dari jumlah seluruh kasus trauma. Blunt Abdominal Trauma Scoring System (BATSS) memberikan akurasi tinggi dalam mendiagnosis cedera organ intra-abdomen pada pasien trauma tumpul abdomen berdasarkan gambaran klinis seperti riwayat pasien, pemeriksaan sik dan FAST. Penelitian uji diagnostik untuk mengetahui validitas nilai diagnostik skor BATSS pada kasus trauma tumpul abdomen.

Bahan dan Metode: Penelitian ini dilaksanakan di RSUP Sanglah Denpasar dengan jumlah sampel sebanyak 44 penderita. Dengan populasi penderita trauma tumpul abdomen yang memenuhi kriteria inklusi dan eksklusi. Desain penelitian ini adalah cross-sectional.

Hasil: Pada 44 penderita, skor BATSS 312 terdapat pada 34 (77,3 %) dan skor BATTS <12 sebanyak 10 (22,7 %). Untuk kelompok dengan skor BATSS 312, terdapat 32 (94,11%) penderita, sedangkan untuk kelompok dengan skor BATSS <12, sebanyak 3 (30%) penderita dengan ruptur organ. Uji validitas yang dilakukan terhadap skor BATSS didapatkan sensitivitas 91,4%, spesi sitas 77,77%, nilai prediksi positif 94,1%, angka prediksi negatif 70%.Skor BATSS dapat membantu sebagai alat identi kasi awal dan strati kasi pasien yang berisiko tinggi terjadinya cedera organ intra-abdomen akibat trauma tumpul abdomen.

Kesimpulan: Skor BATSS dapat membantu sebagai alat identi kasi awal dan strati kasi pasien yang berisiko tinggi terjadinya cedera organ intra-abdomen akibat trauma tumpul abdomen.

 

Introduction: Abdominal trauma is one of the third causes of death in trauma patients and can be found in about 7-10% of the total number of trauma cases. The Abdominal Blunt Trauma Scoring System (BATSS) provides a high-accuracy score system for diagnosing injury to intra-abdominal organs in abdominal blunt trauma patients based on clinical features such as patient history, physical examination and FAST. This study is a diagnostic test study to determine the validity of diagnostic value of BATSS score in cases of blunt abdominal trauma.

Method: This research was conducted at RSUP Sanglah Denpasar with a total sample of 44 patients. With a population of abdominal blunt trauma patients who meet the inclusion and exclusion criteria. The design of this study was cross sectional.

Result: Of the 44 patients, BATSS score 3 12 was found at 34 (77.3%) and BATTS <12 scores of 10 (22.7%). For groups with BATSS score 312, there were 32 (94.11%) patients. As for the group with BATSS score < 12, as many as 3 (30%) patients with organ rupture. Validity test of BATSS score obtained 91.4% sensitivity, 77.77% speci city, positive predictive value 94.1%, negative predictive value 70%. BATSS can be a tool of early identi cation and strati cation of patients at high risk of the occurrence of intra-abdominal organ injury due to blunt abdominal trauma.

Conclusion: BATTS score can help to identify and to do strati cation to patient with high risk of intra-abdominal injury because of abdominal blunt trauma.
