Menopause is women final menstrual period, the period of time before that point is known as premenopause, and 12 months after that point is known as postmenopause. In postmenopausal women, estrogen  level will decrease and there will be vaginal integrity disorder due to decreasing collagen and elastin level which are the part of extracellular matrix. Previous studies have discussed about the correlation of collagen and elastin expression with the prevalence of pelvic organ prolaps but only few that discussed its correlation to menopause state in which the level of estrogen is decreasing. Objective, to determine correlation of estradiol level with collagen and elastin expression in premenopausal and postmenopausal vagina. This is a cross-sectional study. Samples are patients who did a total hysterectomy with indication pelvic organ prolapse and benign gynecologic abnormalities. Twenty four samples included 10 premenopause patients and 14 postmenopause patients. Patient was examined for estradiol serum  levels and expression of vaginal collagen and elastin. Pearson correlation test was used  to determine the relationship between estradiol levels with expression of collagen and elastin vagina. Patient mean age was 51.21 ± 9.31 years with range of 35 to 67 years, mean of parity was 2.75 ± 1.15 with range  0 to 5, and mean of body mass index was 23.52 ± 2 , 78 with range  21.09 and 32.4. Correlation of estradiol level with collagen expression in premenopausal and postmenopausal vagina r=0,67; p=0,001. Correlation of estradiol level with elastin expression r=0,73; p=0,001.   The results of this study concluded that estradiol  level correlate with collagen and elastin expression  in premenopausal and postmenopausal vagina.
