Karakteristik pasien proptosis di poliklinik mata divisi rekonstruksi, okuloplasti, dan onkologi Rumah Sakit Umum Pusat Sanglah, Denpasar

Abstract

Pendahuluan: Proptosis merupakan manifestasi klinis dari suatu patologi yang mengakibatkan perubahan posisi bola mata lebih ke anterior, dapat disebabkan oleh karena penyakit didalam struktur atau sekitar orbita, dan akibat dari beberapa penyakit sistemik. Penelitian ini bertujuan untuk mengetahui karakteristik pasien proptosis di Poliklinik Mata Divisi Rekonstruksi, Okuloplasti, dan Onkologi (ROO) RSUP Sanglah Denpasar. 

Metode: Penelitian observasional dengan pendekatan studi potong lintang dilakukan, dengan mengumpulkan data secara retrospektif berdasarkan rekam medis pasien proptosis di Divisi ROO Poliklinik Mata RSUP Sanglah periode Januari 2017 hingga Juni 2018. Analisis data sampel dengan SPSS for windows versi 24.0 dan data disajikan dalam bentuk tabel dan narasi. Hasil: Subjek penelitian berjumlah 97 orang dan 112 mata dengan proptosis, terdiri dari 42 (43,3%) lelaki dan 55 (56,7%) perempuan. Mayoritas subjek adalah kelompok umur 36-50 tahun (midlife), dan tajam penglihatan mild vision loss (<0.1 dan ≥ 0.5) (34,8%). Gejala mata menonjol dan penglihatan kabur yang paling banyak dikeluhkan. Subjek lebih banyak mengalami proptosis axial 51(52,6%), dan neoplastik menjadi etiologi terbanyak yaitu 55 orang (56,7%).

Diskusi: Sejumlah 28 pasien mengalami thyroid eye disease, sebagai penyebab terbanyak pada kelompok etiologi inflamasi. Kasus proptosis bilateral seluruhnya akibat non-neoplastik dan lebih banyak mengalami tipe axial. 

Kesimpulan: Kasus non-neoplastik lebih sering ditangani secara konservatif, sedangkan tindakan operasi lebih banyak akibat kasus neoplastik, p<0,05

Introduction: Proptosis is a clinical manifestation that results in change of eyeball position anteriorly, usually caused by conditions in the orbit or surrounding structures, and as result of several systemic diseases. Present study aimed to describe the characteristics of proptosis patients in the Eye Polyclinic of ROO Division at Sanglah General Hospital, Denpasar. 

Methods: Observational study with a cross-sectional study approach, data collected retrospectively based on medical records in the eye clinic of ROO Division at Sanglah Hospital for the period January 2017 to June 2018. Data were analyzed by SPSS 24.0 version and presented with table and naration. 

Results: The subjects were 97 patients and 112 proptosis eye, consisting of 42 (43.3%) men and 55 (56.7%) women. The majority of subjects were 36-50 year age group (midlife), with mild vision loss (<0.1 and ≥ 0.5) (34.8%). Protrusion of eye and blurred vision were the most prominent complain. Axial proptosis was the most common type (52,6%), and the most common etiology was neoplastic (56,7%). 

Discussion: Twenty eight subjects had thyroid eye disease, as the most common etiology in inflammation group. Bilateral proptosis cases are entirely due to non-neoplastic cause and mostly with axial types. 

Conclusion: Non-neoplastic cases are mostly treated conservatively, whereas surgical procedures are mostly due to neoplastic, p<0.05.
