Efek pemberian antibiotika profilaksis terhadap hasil kultur tube thoracostomy pada pasien pneumothoraks akibat trauma tumpul thoraks di RSUP Sanglah Denpasar periode September 2018

Abstract

Latar Belakang :Trauma thoraks merupakan salah satu jenis trauma dengan insiden tinggi yang menyebabkan morbiditas dan mortalitas setelah trauma kepala dan ektremitas. Tindakan pemasangan selang torakostomitelah menjadi salah satu standar manajemen trauma thoraks. Infeksi merupakan komplikasi yang dapat terjadi selama perawatan yang telah diperdebatkan karena keterkaitannya dengan penggunaan antibiotika profilaksis. Hingga saat ini belum ada pedoman penggunaan antibitiotika profilaksis pada pemasangan selang torakostomipada kasus pneumothorak. Oleh karena itu, peneliti tertarik meneliti efek pemberian antibiotika profilaksis terhadap hasil kultur selang torakostomi pasien pneumothoraks akibat trauma tumpulthoraks.

Metode: Rancangan penelitiandengan menggunakan Double Blind Randomized Post Test Only Control Group Design.Subyek penelitian adalah pasien trauma tumpul thoraksdengan pneumothorak syang datang ke instalasi gawat darurat  Rumah Sakit Umum Pusat Sanglah  memiliki indikasi pemasangan selang torakostomi . Sebesar 30 pasien yang termasuk dalam sampel penelitian kemudian dilakukan perbandingan terhadap hasil kultur pada ujung selang  antara pasien yang mendapat antibiotika profilaksis dan plasebo.

Hasil: Tidak ada perbedaan hasil kultur antara sampel yang mendapat antibiotika profilaksis dengan sampel yang mendapat plasebo pemberian antibiotika terhadap hasil kultur tubethoracostomy (p=1,00).  Pemberian antibiotika, umur, fraktur kosta, dan lama pemakaian TT tidak mempunyai pengaruh terhadap hasil kultur tubetorachostomy diatas dengan p> 0,05. Analisis ini juga menunjukkan bahwa diberikan atau tidaknya antibiotik dengan mengontrol usia, kondisi fraktur kosta, dan durasi TT tidak bermakna terhadap kejadian infeksi pada pemasangan torakostomi WSD (OR yang mencakup angka 1).

Simpulan: Tidak terdapat perbedaan hasil kultur selang thorakostomi antara yang diberikna antibiotika profilaksis dan plasebo pada pasien pneumothoraks akibat trauma tumpul thoraks yang memerlukan pemasangan TT di Rumah Sakit Sanglah Denpasar.

 

Abstract

Introduction: Thoracic trauma is one type of trauma with a high incidence that causes morbidity and mortality after head trauma and extremity. The act of installing a thoracostomy hose has become one of the standards for management of thoracic trauma. Infection is a complication that can occur during treatment that has been debated because of its association with the use of prophylactic antibiotics. Until now there has been no guideline for the use of antibitiotika prophylaxis in the installation of a thoracostomy hose in pneumothoracic cases. Therefore, researchers are interested in examining the effect of prophylactic antibiotics on the results of thoracostomy hose culture of pneumothorax patients due to blunt thoracic trauma.

Method: Research design using Double Blind Randomized Post Test Only Control Group Design. The subjects of the study were blunt thoracic trauma patients with pneumothorax who came to the emergency department Sanglah Central Hospital had an indication of the installation of a thoracostomy hose. As many as 30 patients included in the study sample then compared the results of culture at the end of the interval between patients who received prophylactic antibiotics and placebo.

Results: There were no differences in culture results between samples receiving prophylactic antibiotics and samples receiving placebo antibiotics for thoracostomy tube culture results (p= 1.00). The administration of antibiotics, age, rib fracture, and duration of TT use did not have an effect on the results of the torachostomy tube culture above with p-value > 0.05. This analysis also showed that antibiotics were given or not by controlling age, costal fracture conditions, and the duration of TT was not significant for the incidence of infection in the installation of WSD thoracostomy (OR which includes 1).

Conclusion: There were no differences in the results of the interval thoracostomy culture that was given prophylactic antibiotics and placebo in pneumothorax patients due to blunt thoracic trauma that required the installation of TT at Sanglah General Hospital.
