Two cases of type 2 diabetes mellitus (T2DM) in children

Abstract

Increasing prevalence of obesity in children and adolescents, also making a high prevalence of Type-2 Diabetes Mellitus (T2DM). T2DM initially said to be an adult disease is now reported in children and adolescents in the developed countries because of increased incidence of obesity and sedentary habits associated with lifestyle changes.  We report two cases of T2DM with onset at or below 12 years of age who registered at Sanglah General Hospital, Denpasar, Bali. T2DM was diagnosed based on the absence of ketosis, good beta cell reserve as shown by the C peptide assay, absence of insulin autoantibody, and response to oral hypoglycemic agents. The objective of these reports is to describe clinical characteristics, management, and social aspects for T2DM patients.  We report two cases of T2DM at the age of 10 and 12 years old. Both patients were female. We evaluate the patient by visiting them to look at their social aspect and problem also management. All of them initially have the same problem in compliance to the medication and lifestyle modification, both of them have a good psychological aspect found using PedsQl, They also have a family history of diabetes but only one is an obese patient. T2DM is now beginning to be seen in the first decade of life. Screening for children who have a risk of diabetes is important. A complication is the biggest fear for T2DM patient, especially for children, routine management and regular evaluation should be done.

 

Meningkatnya prevalensi obesitas pada anak-anak dan remaja, juga membuat meningkatnya angka prevalensi pada anak dengan Diabetes Mellitus Tipe-2 (DMT2). Diabetes Mellitus tipe 2 awalnya suatu penyakit yang sering didapatkan pada orang dewasa, namun sekarang sering dilaporkan mulai terjadi pada anak-anak dan remaja di negara maju karena peningkatan kejadian obesitas dan kebiasaan yang menetap terkait dengan perubahan gaya hidup tidak sehat. Kami melaporkan dua kasus DMT2 dengan onset usia di bawah 12 tahun yang terdaftar di Rumah Sakit Umum Sanglah, Denpasar, Bali. Kami melaporkan dua kasus DMT2 pada usia 10 dan 12 tahun. Kedua pasien adalah perempuan. Kami mengevaluasi pasien dengan melihat aspek sosial dan masalah mereka serta tatalaksana DMT2. Diabetes Mellitus tipe 2 sekarang mulai terlihat pada dekade pertama kehidupan. Skrining untuk anak-anak yang memiliki risiko diabetes adalah penting untuk mencegah terjadinya DMT2. Komplikasi merupakan masalah terbesar bagi pasien DMT2, terutama untuk anak-anak, serta melakukan pemantauan rutin dan evaluasi reguler sebaiknya harus tetap dilakukan.
