Varikokelektomi untuk memperbaiki parameter sperma pada pasien infertil dengan varikokel


Background. Varicocele is one of the causes of male infertility that can be corrected surgically. Varicocellectomy can be considered in adult men with clinical varicoceles with abnormal sperm analysis, infertile ≥ 2 years, and in unexplained infertility, to improve sperm parameters and male fertility potential. This study aims to determine the rate of improvement in the quality of post-varicocellectomy sperm parameters in infertile patients with varicoceles.

Method. This was a prospective cohort observational analytic study. Samples were taken from the medical records of patients who underwent varicocelectomy surgery from January 2016 to February 2019. The study involved 100 research subjects, namely grade 2-3 varicocele patients and surgery performed by the same operator. The difference evaluated in the form of postoperative sperm parameters in the form of sperm concentration, sperm motility, and normal sperm morphology.

Results. In this study, there were significant differences between pre and post varicocellectomy sperm parameters, both sperm concentration, sperm motility, and normal sperm morphology, with p <0.001. The overall success rate, out of 100 subjects, found an increase in sperm concentration in 85 patients (85%), an increase in sperm motility in 73 patients (73%), and an increase in normal sperm morphology in 65 patients (65%)

Conclusion. Varicocellectomy can significantly improve sperm parameters in infertile patients with varicoceles, both sperm concentration, sperm motility, and normal sperm morphology.


Latar belakang. Varikokel merupakan salah satu penyebab infertilitas pada pria yang dapat dikoreksi dengan pembedahan. Varikokelektomi dapat dipertimbangkan pada pria dewasa dengan varikokel klinis dengan analisis sperma abnormal, infertile ≥ 2 tahun, dan pada infertil yang tidak bisa dijelaskan, untuk meningkatkan parameter sperma dan potensi fertilitas pria. Studi ini bertujuan untuk mengetahui angka perbaikan kualitas parameter sperma paska varikokelektomi pada pasien infertil dengan varikokel.

Metode. Penelitian ini menggunakan rancangan penelitian observasional analitik kohort prospektif. Sampel diambil dari rekam medis pasien yang menjalani operasi varikokelektomi dari Januari 2016-Februari 2019. Penelitian ini melibatkan 100 pasien varikokel grade 2-3 dan operasi dikerjakan oleh operator yang sama. Perbedaan yang dievaluasi berupa parameter sperma pasca operasi berupa konsentrasi sperma, motilitas sperma, morfologi sperma normal.

Hasil. Pada penelitian ini, didapatkan perbedaan yang bermakna, antara parameter sperma pre dan pasca varikokelektomi, baik pada konsentrasi sperma, motilitas sperma, maupun morfologi sperma normal, dengan p <0,001. Tingkat kesuksesan secara keseluruhan, dari 100 subyek, didapatkan peningkatan konsentrasi sperma pada 85 pasien (85%), peningkatan motilitas sperma pada 73 pasien (73%), dan peningkatan morfologi normal sperma pada 65 pasien (65%)

Kesimpulan. Varikokelektomi dapat memperbaiki parameter sperma secara signifikan pada pasien infertil dengan varikokel, baik konsentrasi sperma, motilitas sperma, maupun morfologi sperma normal.
